Job Overview
South Carolina Department of Social Services is seeking a Case Manager for Adult Protective Services (APS) in Lee County and surrounding counties.
Job Responsibilities- Perform specialized case management in APS, including investigations, assessments, and coordination of protective and preventive services for vulnerable adults.
- Manage an assigned caseload of vulnerable adults, conducting client contact, home visits, service planning, and coordination of legal actions as appropriate.
- Conduct timely assessments to determine risk factors such as abuse, neglect, self‑neglect, and exploitation, and implement protective interventions while honoring client choice.
- Maintain accurate, objective, and timely documentation in electronic case systems.
- Develop and coordinate service plans in partnership with clients to promote safety and stability, arranging placements and community‑based supports.
- Collaborate with law enforcement and the court system to support emergency protective actions and assist clients in securing appropriate housing or protective arrangements.
- Coordinate communication among clients, families, providers, and internal staff to support effective service delivery.
- Assess clients’ capabilities to access long‑term supports, including financial resources, guardianship, and court involvement.
- Support emergency response operations, including Red Cross sheltering and disaster‑related services.
- Perform other duties as required and/or directed.
- Bachelor's Degree in Social Work, Behavioral Science, or Social Science.
- Bachelor's Degree in any other field and one year of experience in a related field.
- Must possess a valid driver's license and have access to a motor vehicle for field work.
