Care Lync seeks a Supports Coordinator for Early Intervention Services to coordinate care for children birth to age 6 and their families. You will be the primary contact, help families access services, and ensure coordinated, high-quality supports that promote development and family capacity.
The role covers assessment, planning, family advocacy, service monitoring, documentation, and collaboration with teams across systems. Travel and community-based work are involved.
