Care Lync is seeking a Supports Coordinator (Early Intervention Services) to provide ongoing service coordination for children birth to age 6 and their families. You will ensure access to services, explain rights, and coordinate comprehensive supports to promote child development and family capacity.
You will lead evaluations, IFSP/FSP development, and team collaboration, while maintaining accurate documentation and ensuring compliance with Medicaid and IDEA Part C requirements.
