Case Manager; LOTUS

Position: Case Manager (LOTUS)
Location: Dunean

Under the general supervision of the Serenity Village Client Care Coordinator this position provides a wide range of duties related to _case management to women/families enrolled in the Serenity Place and the Lotus programming. This position will require coordination of case management services with other agencies and service providers.

JOB DUTIESWill complete orientation paperwork with new Lotus clients prior to their moving to the villa.

Will complete and organize and approval passes for Lotus clients with Lotus team input.

Will complete villa inspections weekly with Operations staff.

Will have weekly home visits with all villa clients.

Will aid clients in applying for financial benefits like Medicaid, TANF/FI, SNAP and ABC vouchers.

Completes Targeted Case Management assessments and TCM plans as assigned.

Organizes and schedules appointments for community resources (WIC, Social Security, etc.)Promote services to include phone support, referrals and linkage to community services as needed.

Coordinates and monitors women/families to obtain needed community services in connected to case management assessment and treatment plan.

Conduct weekly home visits with women/families.

Handles and documents any crisis situations with Lotus clients.

Attends all case management meetings and keeps staff informed of care coordination needs.

Evaluates client’s progress on a regular basis with input from client and the treatment team.

Keeps Lotus team informed on caseload status and clients’ progress.

Maintains all appropriate documentation of services.

Transports residents to appointments and other activities off-site as needed.

Performs any other related duties as required.

Experience and training in working with special populations (women, children, minorities) providing case management services

Must have valid SC driver’s license and be able to provide proof of insuranceREQUIRED TRAINING AND EXPERIENCEA bachelor’s degree with two years case management experience preferred. Prior experiences with AOD population(s) and women’s services.
