• EDUCATION, CERTIFICATION, AND/OR LICENSURE: Certified Diabetes Care and Education Specialist (CDCES) through the Certification Board for Diabetes Care and Education
• EXPERIENCE: One (1) year experience in health and fitness

• EDUCATION, CERTIFICATION, AND/OR LICENSURE: ASCM, Clinical Exercise Physiology Certified

• Identifies education needs of the community and those with diabetes.
• Attends diabetes advisory committee meetings as assigned.
• Performs patient assessment, delivers education and evaluates patient's attainment of goals.
• Assists in maintaining the American Diabetes Association certification as applicable.
• Participates in community outreach and educational support.
• Advocates on behalf of the patient and families.
• Communicates with primary care providers, members of the multi-disciplinary team regarding relevant questions, concerns, and patient outcomes.
• Participates in performance improvement activities to assure quality care.
• Provides administrative support and coordinates the management and operations of the Diabetic Institute.
• Leads the planning, implementation, and evaluation of the Outpatient Diabetes and Nutrition Education Program.
• Compiles reference materials, summarizes data and prepares reports, and drafts manuals and materials.

• Ability to lift 25 pounds.
• Ability to push/pull 100 pounds.
• Ability to stand for several hours per day.
