MINIMUM QUALIFICATIONS- EDUCATION, CERTIFICATION, AND/OR LICENSURE: Certified Diabetes Care and Education Specialist (CDCES) through the Certification Board for Diabetes Care and Education
- EXPERIENCE: One (1) year experience in health and fitness
- EDUCATION, CERTIFICATION, AND/OR LICENSURE: ASCM, Clinical Exercise Physiology Certified

- Identifies education needs of the community and those with diabetes.
- Attends diabetes advisory committee meetings as assigned.
- Performs patient assessment, delivers education and evaluates patient's attainment of goals.
- Assists in maintaining the American Diabetes Association certification as applicable.
- Participates in community outreach and educational support.
- Advocates on behalf of the patient and families.
- Communicates with primary care providers, members of the multi-disciplinary team regarding relevant questions, concerns, and patient outcomes.
- Participates in performance improvement activities to assure quality care.
- Provides administrative support and coordinates the management and operations of the Diabetic Institute.
- Leads the planning, implementation, and evaluation of the Outpatient Diabetes and Nutrition Education Program.
- Compiles reference materials, summarizes data and prepares reports, and drafts manuals and materials.
- Ability to lift 25 pounds.
- Ability to push/pull 100 pounds.
- Ability to stand for several hours per day.
- Wellness Center
- Computer skills, particularly Excel and Word.
- Ability to understand written and oral communication.
