Job SummaryThe Clinical Care Leader (CCL) is responsible for utilizing the nursing process to provide individualized nursing care in the home setting. The role collaborates with inter‑professional colleagues to plan, implement, and evaluate care; documents resident history, symptoms, medications and care provided in compliance with applicable standards; utilizes technology to enhance quality resident care and collaborates with team members; effectively delegates to and supervises unlicensed personnel;
assumes overall nursing accountability for a resident assignment; serves in leadership roles on department/organizational task forces and committees; demonstrates competency and practices within the full scope of nursing expertise/knowledge using appropriate age and population‑specific standards.
Must have effective communication skills to collaborate, negotiate, and resolve conflicts with peers, inter‑professional colleagues, residents, and families. Must possess strong clinical reasoning and critical thinking skills to analyze data and respond appropriately to changing situations. Work includes coordination of care, resident assessment, resident education, triage, and various nursing interventions.
Functions within the scopes and standards of nursing practice as outlined in the Nurse Practice Act and Administrative Rules. The Sanford Professional Nursing Practice recognizes the Scope and Standards of Practice and the Code of Ethics for Nurses.
Qualifications- Bachelor's degree in Nursing strongly preferred (graduate from an accredited nursing program accredited by CCNE, ACEN, or NLN CNEA).
- Minimum of two years’ clinical experience preferred.
- Current unencumbered RN license with the State Board of Nursing where practice occurs; may hold multistate licensure in a Nurse Licensure Compact state.
- Obtains and maintains required department‑specific competencies and certifications; may require a valid driver's license depending on location. Basic Life Support (BLS) required.
- Resident Assessment Instrument (RAI) process experience preferred, but not required.
