• Provide professional nursing care to patients in their home.
• Assess, develop, implement, and evaluate home nursing care needs.
• Ensure compliance with physician orders and documentation.
• Demonstrate holistic care for patients and their significant others.
• Complete nursing assessments and medication reconciliations.
• Update care plans within two weeks of changes in patient status.
• Act as a liaison among physicians, management, and healthcare team members.
• Supervise para‑professional home health aide staff.
• Maintain awareness of new standards and incorporate them into practice.
• Complete routine assignments accurately and independently.

Associate's Degree in Nursing or Diploma Certificate in Nursing.

At least six (6) months RN experience or at least one year of LPN experience, preferably in home care or hospice.

• NJ State Professional Registered Nurse License
• AHA Basic Health Care Life Support HCP Certification
