Position Title: Clinic LPN/RN

Facility: Sisseton Clinic Primarily –

Required to provide coverage at Rosholt and Browns Valley Clinics as needed

Reports to: Clinic Nurse Manager

Classification: Full Time – 40hr/week

Shift: 8 hr day (8:00am-5:00pm)

Pay Range:

LPN $22.00-$30.80

RN $27.00-$39.15

Summary of Position

The nurse will provide professional, high‑quality nursing care in a clinic setting in accordance with physician orders, established policies, and scope of nursing practice. Responsibilities include patient intake and rooming, obtaining and documenting vital signs and clinical history, performing clinic‑based nursing procedures, administering medications as ordered, and coordinating care, referrals, and follow‑up to support patients and families throughout their visit.

Essential Duties and Responsibilities
  • Prepare patients for examinations by obtaining and documenting vital signs, chief complaints, and relevant health history.
  • Review and update patient charts including medical history, medications, allergies, and immunization status in the electronic health record (EHR).
  • Assist providers during examinations and procedures, including room setup, patient positioning, and maintaining a sterile field as needed.
  • Perform clinical procedures within scope, such as wound care, dressing changes, suture removal, ear irrigation, nail care, and basic vision/hearing screenings.
  • Administer medications and injections as ordered, including IM and subcutaneous injections, immunizations, allergy injections, and PPD/TB testing.
  • Perform CLIA-waived point‑of‑care testing and accurately document and report results (e.g., glucose, urinalysis, rapid tests).
  • Conduct pediatric support, including weights, vital signs, developmental screening, growth measurements, and well‑child data collection.
  • Triage and respond to incoming patient phone calls, assess urgency, provide education per protocol, and route concerns appropriately to providers.
  • Coordinate care and follow‑up, including scheduling referrals, relaying test results, tracking orders, and communicating with pharmacies under provider direction.
  • Provide patient education related to treatments, medications, procedures, chronic disease management, and preventive care.
  • Maintain exam rooms and clinical equipment, ensuring cleanliness, stocking supplies, and proper disinfection and sterilization.
  • Support rural clinic workflows, assisting across multiple service areas and adapting to variable patient needs and staffing demands.
  • Ensure patient safety and confidentiality, complying with HIPAA, infection control standards, and clinic policies.
  • Participate in quality improvement, staff meetings, and mandatory training, maintaining licensure, BLS certification, and required competencies.
  • Collaborate effectively with the healthcare team, including providers, nurses, ancillary staff, and external agencies, to ensure continuity of care.
