Horizon Health in Wessington Springs, SD seeks a full-time Nurse Practitioner or Physician Assistant to provide direct patient care for all patients. • Emergency Room and Hospital coverage required with weekday and weekend rotation.
• Required ACLS, ATLS, and PALS certifications or willingness to obtain upon hire.
• Licensed or able to obtain licensure in the State of South Dakota. Certification from an accredited organization needed or obtained prior to employment.

Base salary provided, with additional compensation for ER call.

• Provides primary health care to patients, according to established evidence-based guidelines of clinical practice.
• Lead care teams to provide high quality healthcare in accordance with Horizon Quality Improvement Plan and meet established goals and benchmarks.
• Management of long-term care for acute and chronically ill patients, including care coordination and development of care plans and self-management plans.
