Outpatient Staff Registered Nurse
Make a meaningful impact on the health and independence of Veterans as an Outpatient Staff Registered Nurse at the VA Black Hills Health Care System in Fort Meade. In this collaborative role, you will build trusted relationships with Veterans and their caregivers while supporting preventive care, chronic disease management, and informed health decisions. You will work alongside an interdisciplinary team to coordinate services, develop individualized care plans, and promote continuity across the care journey.
This position offers an excellent opportunity to strengthen your clinical practice, expand your professional skills, and contribute to high-quality, Veteran-centered outpatient care. Duties include, but are not limited to:
- Assess Veterans' health needs and develop individualized nursing care plans aligned with their goals, preferences, and clinical conditions.
- Deliver and evaluate outpatient nursing care using the nursing process to support safe, effective, and coordinated treatment.
- Coordinate care across primary care, specialty services, and other members of the interdisciplinary team to promote continuity and positive outcomes.
- Educate and coach Veterans and caregivers on wellness, disease prevention, self-management, and chronic condition management.
- Administer medications, treatments, and procedures in accordance with established clinical guidance and individualized care plans.
- Build therapeutic relationships with Veterans and families by incorporating their values, beliefs, and preferences into shared care decisions.
- Delegate appropriate elements of care based on each Veteran's assessed needs, condition, and safety considerations.
- Apply evidence-based practices and participate in unit-level improvement activities that strengthen care quality and resource utilization.
