- Responsible for the oversight of General Inpatient Hospice services at multiple HMH Hospitals.
- Provides collaboration in developing a plan of treatment with physicians to ensure the patient meets GIP criteria for admission.
- Responsible for a 24 hour audit of the active GIP census to ensure criteria are met. Anticipates level of care change should the patient no longer meet GIP criteria and utilizes appropriate hospice team resources to facilitate a discharge plan ( ie home, nursing facility).
- Acts as a resource to hospital staff (nurses, case managers, physicians) in providing ongoing hospice education. Able to recognize process improvement opportunities and make actionable recommendations.
- Conducts regular meetings with hospice GIP nurses; participates in recruitment, orientation, and competencies of staff. Ensures compliance with their annual mandatory training. Ensures appropriate staffing and maintains vacancy and turnover metrics at established goals.
- Builds and maintains excellent relationships with referral sources and hospital stakeholders. Promotes and represents HMH Hospice at hospital committees and meetings.
- Recognizes grievances of patients, families, physicians, and/or hospital stakeholders and interacts in an appropriate, constructive manner. Surfaces issues to leadership and takes ownership of overall resolution.
- Other duties and/or projects as assigned.
- Adheres to HMH Organizational competencies and standards of behavior.
- Performs direct patient care visits, in the hospital or field setting, should the need arise.
Education,
Knowledge, Skills and Abilities Required:
- Associate's degree in Nursing.
- Minimum of three (3) years of RN experience.
- Excellent written and verbal communication skills.
- Proficient computer skills that include but are not limited to Microsoft Office and/or Google Suite platforms.
Skills And Abilities
Preferred
- BSN in progress or willing to acquire within three (3) years of hire.
- Two (2) to three (3) years of management experience.
- Hospice experience.
- NJ State Professional Registered Nurse License.
- AHA Basic Health Care Life Support HCP Certification.
