Job Title

The incumbent is a member of the outpatient and inpatient Palliative and Hospice Care teams for BHHCS.

The expected duties and responsibilities to include but are not limited to the following:

• Use a high level of skill in assessing and treating the complicated psychosocial problems of Veterans and their families/significant others.
• Conduct psychosocial assessments to determine needs of veterans and family members.
• Be an active participant in the treatment planning process with other disciplines.
• Coordinate and refer care with other disciplines and ensure that the treatment plan is executed in a manner that is timely and appropriate.
• Provide consultation/education to Veterans and families on community resources, advance directives and VA benefits.
• Act as a consultant to other team/staff members.
• Provide consultation/education to medical residents on social work practice and VA policy and procedure.
• Act as liaison between VA and community resources, including marketing and public relation duties as needed.
• Provide a full range of social work services within commonly accepted standards of social work practice which includes casework, individual, family and group counseling, discharge planning, case management and other services as appropriate throughout the continuum of care.
• Address Advance Directives, Goals of Care, and Organ Donation conversations and related activities in accordance with the standards of their work area.
• Provide wellness/prevention education and facilitate inpatient/family support groups as appropriate.
• Supervise the practice of social work students, or a less experienced social worker.
• Coordinate treatment and communicate with the care lines to facilitate continuity of care for veterans.
• Assist the Veteran and family in understanding contributing factors to problems, discuss solutions, and encourage lasting changes to reduce stressors.
• Participate in committee assignments, as deemed necessary by the service manager.
• Maintain data and statistical compilations to comply with JCAHO, VA and medical center policy and procedures.
• Assist patients and their significant others with coping and dealing with the loss and grief experiences in disability, terminal illness and death.
• Assess and manage for suicide risk, patient safety and risk, especially as it relates to vulnerable adults.
• Document social work interventions and activities in patients' electronic clinical record and ensure appropriate hand off when transferring of patient care.

The incumbent demonstrates the capacity to expand clinical knowledge in the profession, provide consultation and guidance to colleagues and stakeholders, and model effective social work practice skills. Apart from providing consultation and case management on difficult and complex clients, the incumbent consults with medical providers and team members on achieving positive outcomes for the Veteran and the facility. Additionally, the incumbent may represent social work and social service issues in care line, facility or community committees and work groups.

Monday-Friday 7:30am-4pm

37-50 days of annual paid time offer per year (13-26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year)

After 12 months of employment, up to 12 weeks of paid parental leave in connection with the birth, adoption, or foster care placement of a child.

After 60 days of employment, full time employees with a total family income below $144,000 may be eligible for a childcare subsidy up to 25% of total eligible childcare costs for eligible children up to the monthly maximum of $416.66.
