Position: Family Nurse Practitioner / Physician's Assistant (Tues - Sat)

Job Summary

At Via Care Community Health Center (VCCHC), our mission is to provide exceptional health care services to patients throughout their lives, with a vision of ensuring that affordable, high‑quality care is accessible to all. The Family Nurse Practitioner or Physician’s Assistant (FNP/PA) will work collaboratively with physicians and other health care professionals to provide comprehensive medical services to patients regardless of age or gender (as allowed by training/experience).

Services include preventive care, immunizations, primary care and treatment, health education, family planning, reproductive health services, physical examinations, and more. The FNP/PA will practice under a supervising physician.

Position is Full‑Time
, working Tuesday – Saturday

Duties & Responsibilities

The FNP/PA performs medical histories, physical exams, assessment and planning of clinic patients based on chief complaint and/or health status, and documents Health Care Maintenance status as per established protocols. In addition, the FNP/PA:

  • Carries out mission, vision and values established by the CEO/Chief Medical Officer and Board of Directors
  • Performs and/or assists with laboratory and screening procedures delegated by the supervising physician
  • Performs or assists with therapeutic procedures delegated by the supervising physician
  • Recognizes and evaluates situations that call for immediate attention of the physician
  • Instructs and counsels patients regarding matters pertaining to their physical and mental health
  • Facilitates patient referrals to appropriate health facilities, agencies, and community resources
  • Reviews treatment and therapy plans, orders routine diagnostic laboratory tests and procedures, and routine diagnostic radiological services
  • Performs other duties as assigned by supervisor
