We are seeking a dedicated RN – MDS/Care Plan Coordinator to ensure accurate resident assessments and up-to-date care plans
. This position is responsible for maintaining detailed documentation of resident care
, ensuring compliance with state and federal regulations, and coordinating care between residents, families, and the interdisciplinary team
.

RN – MDS/Care Plan Coordinator Requirements:
- Current State of Illinois RN license
- Ability to read, write, and speak English fluently
- AANAC certification preferred
- MDS experience required
- Strong working knowledge of nursing services, administration, rehabilitation, geriatric care, and MDS documentation (EMR systems)
- Strong managerial, organizational, and planning skills
- Ability to initiate, complete, and update care plans
- Experience conducting staff training on care plans, documentation, and EMR use
- Ability to monitor EMR records for consistency and compliance
- Strong interpersonal skills to communicate with residents, families, and staff professionally
- Compassionate, detail-oriented, and committed to high-quality resident care
- Knowledge of JCAHO, OBRA, IDPH, and HFS documentation standards
As an RN – MDS/Care Plan Coordinator, you will:
- Coordinate and complete MDS assessments upon admission, readmission, quarterly, annually, and during significant changes or PPS/Insurance Reimbursement periods
- Maintain and provide a monthly calendar for scheduled MDS assessments and Care Plan Conferences
- Ensure timely completion of MDS sections by the appropriate department and validate accuracy
- Identify and address MDS-related issues
, providing re‑education to staff when necessary - Review diagnosis coding and sequencing with physicians quarterly, ensuring accurate ICD‑9/ICD‑10 coding
- Lead weekly MDS Pre‑Planning and Medicare meetings for timely and accurate documentation
- Support and educate staff on EMR charting, documentation, and achieving care plan goals
- Communicate resident care plans and progress to staff, residents, and families
- Conduct quarterly in‑service training for nursing staff on care plans and documentation
- Audit monthly EMR charting to ensure proper documentation of care and follow up with staff as needed
- Address documentation discrepancies and ensure compliance with regulatory standards
- Collaborate with the Director of Nursing and other department heads to ensure consistency in resident care
- Coordinate resident Care Plans with therapy departments (physical, occupational, respiratory, and speech therapy)
- Ensure proper nutritional assessments are completed and documented in collaboration with dietary consultants
- Maintain confidentiality of resident information and follow established fire, disaster, safety, infection control, and evacuation policies
