Transfer, transport, and ambulate patients and distribute supplies and equipment
- Current license to practice as a Registered Nurse in state of practice
- Valid compact licenses must be transferred to the state of practice within 60 days of the start of employment
- Basic Life Support (BLS) Certification for healthcare providers
- Bachelor's degree in Nursing (BSN) from an accredited institution; degree will be verified
- RNs with less than 12 months of experience must obtain their BSN within four years of their hire date
- One year of home health or hospice experience strongly preferred
- Experience with hospice admissions, intake, care coordination, case management, or transitions of care preferred
- Familiarity with hospice eligibility criteria and end-of-life care services preferred
- Candidates must reside in Utah
- Rotating days, weekends, holidays, and start times
Demonstrates expertise in hospice care coordination, clinical assessments, and patient support during transitions to hospice services. Holds a valid Registered Nurse license and BLS certification, with a strong background in home health or hospice experience.
