Assist with safe administration of medications and treatments as ordered, following the Five Rights of Medication Administration;
Assist residents with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, eating, and mobility;
Maintain accurate and timely documentation (e.g., change in condition, transfers, STOP & WATCH, electronic systems);
Participate in change-of-shift reports and care coordination;
Communicate effectively with nurses, physicians, and interdisciplinary team members
