In-Home Health - Nurse Practitioner or Physician Assistant; Part Time - RI

Signify Health, part of CVS Health®, is seeking a Part‑time Clinician (Nurse Practitioner or Physician Assistant) to provide In‑Home Health Evaluations. Each visit lasts approximately 45 minutes and may include a medication and medical history review, a physical examination, and optional diagnostic tests ordered by the member’s health plan.

• Conduct virtual visits as required.
• Provide all Signify Health services indicated, including Diagnostic & Preventive Services products.

• Active, unrestricted license(s) in the coverage area(s).
• Board certification (required).
• Multi‑state licenses (preferred).
• Ability to cover the primary location:
  Rhode Island.

Pay ranges from $46.03 to $99.14 per hour, with adjustments based on experience, education, geography and other relevant factors. Additional reimbursement may be available for completed Diagnostic and Preventive Services.

Your primary workplace is the private residence of members. You must be able to safely perform duties in homes that may contain pets, secondhand smoke, allergens, or other potential irritants.

Anticipated weekly hours: 29.5 hours. The role is part‑time with variable daily evaluations.
