In-Home Health - Nurse Practitioner or Physician Assistant; Part Time - RI

Signify Health, part of CVS Health®, is seeking a Part‑Time Clinician (Nurse Practitioner or Physician Assistant) to provide In‑Home Health Evaluations, engaging with people in the comfort of their own homes and helping bridge gaps in care.

• The visit lasts about 45 minutes, includes a medication and medical history review, a physical evaluation, and additional diagnostic tests as ordered by the person’s health plan.
• You’ll use an iPad and an intuitive clinical workflow for each evaluation and are not required to prescribe medication, order lab tests, or alter patients’ current treatment regimen.

• Contribute to a national network of purpose‑driven clinicians dedicated to improving health outcomes.
• Visit millions of patients nationwide, ensuring appropriate connections to care based on their unique needs.
• Work with Signify Health as a clinician colleague, fulfilling role requirements.
• Be flexible to travel locally and to licensed states as needed.
• Conduct virtual visits as required.
• Provide all Signify Health services as indicated, including diagnostic & preventive services such as spirometry, DEE, and bone density screening.

Your primary workplace is the private residence of our members. Candidates must be prepared to safely perform duties in homes that may contain pets, secondhand smoke, and other potential airborne allergens or irritants.

• Active, unrestricted license(s) in the coverage area(s).
• Board certification (required).
• Multi‑state licenses (preferred).
• Ready to cover the following primary location:
  Rhode Island.

Anticipated weekly hours: 29.5 hours.

Time type:
Part time.

Pay range: $46.03 – $99.14 per hour. Base pay is dependent on experience, education, geography and other relevant criteria. This position is eligible for additional reimbursement for completed Diagnostic and Preventive Services.
