You will work within a multidisciplinary team to develop comprehensive, culturally sensitive care plans and to promote patient self-management.
Ideal candidates are licensed RNs in Rhode Island with 2+ years managing complex cases, and experience in chronic disease management. Travel to provider offices may be required.
