Position OverviewJoin our clients dedicated Home Health team as a Physical Therapist
, where you'll make a meaningful difference by helping patients achieve greater independence and improved quality of life—all from the comfort of their own homes. This role offers a supportive clinical environment, a predictable schedule, and the opportunity to build strong relationships with patients and caregivers.
Masters degree or higher in Physical Therapy
Active and unrestricted Physical Therapist (PT) license in the State of Illinois
Minimum of 1 year of experience in a healthcare setting
Valid drivers license and ability to travel locally
PT license number provided at time of application
Conduct comprehensive physical therapy evaluations and develop individualized treatment plans
Provide in-home therapy services focused on improving physical and cognitive functioning
Monitor, document, and report patient responses to treatment and changes in condition
Educate patients and caregivers on therapeutic exercises, safety, and use of adaptive equipment
Supervise Physical Therapy Assistants and, when appropriate, Certified Nursing Assistants
Complete required documentation, prepare clinical reports, and participate in team and agency development activities
Monday–Friday | Day shift (start and end times vary based on patient needs)
One weekend every six weeks (Saturday & Sunday, scheduled visits only)
Two holidays per year (one summer, one winter on a rotating basis)
No on-call requirements
Local travel to residential homes
