Hours: Monday - Friday, 8:30 AM - 5:00 PM (flexibility), 1‑2 weekends per month.

The Fertility Care Patient Navigator is a compassionate, service‑minded role that serves as the primary point of contact for patients, guiding them through their fertility journey and working closely with a multidisciplinary team to ensure exceptional care and support.

• Serve as the primary point of contact for patients, guiding them through their treatment plan and providing education on procedures, testing, and follow‑up care.
• Coordinate and schedule patient appointments, tests, and referrals to ensure a smooth continuum of care.
• Communicate with healthcare providers, clinical staff, and administrative teams to ensure seamless patient care coordination.
• Oversee treatment coordination across departments for individual patient care plans.
• Maintain accurate, up‑to‑date patient records in the electronic medical record (EMR) system.
• Provide emotional support and resources to patients and families, addressing concerns or barriers to care.
• Educate patients on treatment protocols, self‑care strategies, and medication usage.
• Track patient progress, follow up on missed appointments, and ensure compliance with treatment plans.
• Advocate for patients’ needs and help resolve any issues or barriers to care.
• Other duties and projects as assigned.

• Education: High school diploma required. Bachelor’s degree in healthcare, social work, or related field preferred. Certified Medical Assistant Certification (CMA) preferred.
• Experience: Minimum 1‑2 years in customer service, patient navigation, case management, or related healthcare role. IVF/fertility experience and background in women’s health a plus.
• Skills:
  • Strong knowledge of healthcare systems, insurance processes, and patient advocacy.
  • Excellent communication, organizational, and problem‑solving skills.
  • Excellent time and people management skills with the ability to work independently, multitask, and prioritize work.
  • Proficiency in using electronic medical records (EMR) and healthcare software.
  • Empathy and a patient‑focused approach, ability to work collaboratively in a multidisciplinary team.
  • Bilingual skills are a plus.
• Hourly Rate: $24.00 - $26.00 per hour (final offers based on experience, skills and qualifications).
