Responsibilities
  • Provide care and services for a group of patients, visiting patients where they reside, including private homes, skilled nursing facilities and assisted living facilities.
  • Act as a contributing member of the interdisciplinary team, offering expertise regarding psychosocial assessments, interventions and community resources.
  • Work independently managing visits, documentation, and communications while collaborating daily with nurses, social workers, chaplains, volunteers and CNA leadership.
  • Prioritize and triage multiple concerns, responding to urgent needs and scheduling visits.
  • Learn and use the electronic medical record system.
  • Drive within the service area with reliable transportation, a valid driver’s license and auto insurance.
Qualifications
  • Current Registered Nurse (RN) licensure (in good standing) in the state(s) of practice.
  • Strong teamwork and collaboration skills.
  • Excellent time‑management and organizational abilities.
  • Strong communication skills, both written and verbal.
  • Ability to work independently and manage own schedule.
  • Valid driver’s license, reliable transportation and auto insurance.
