This is achieved by allocating resources to increase our staff‑to‑patient ratios, thereby increasing clinical visits while lowering clinician caseloads. - Shift: 3x12 hr – 9 am to 9 pm

- Provide care and services for a group of patients, visiting patients where they reside, including private homes, skilled nursing facilities and assisted living facilities.
- Act as a contributing member of the interdisciplinary team, offering expertise regarding psychosocial assessments, interventions and community resources.
- Work independently managing visits, documentation, and communications while collaborating daily with nurses, social workers, chaplains, volunteers and CNA leadership.
- Prioritize and triage multiple concerns, responding to urgent needs and scheduling visits.
- Learn and use the electronic medical record system.
- Drive within the service area with reliable transportation, a valid driver’s license and auto insurance.
- Current Registered Nurse (RN) licensure (in good standing) in the state(s) of practice.
- Strong teamwork and collaboration skills.
- Excellent time‑management and organizational abilities.
- Strong communication skills, both written and verbal.
- Ability to work independently and manage own schedule.
- Valid driver’s license, reliable transportation and auto insurance.
