As a Weekend In-House Nurse Practitioner
, you will be supporting the primary provider of internal medicine services within a Skilled Nursing Facility (SNF). You will have the opportunity to build strong, lasting relationships with residents, their families, facility staff, and providers, while being an integral part of the clinical leadership team.
At Trio Health Partners, we focus on preventing critical health issues before they arise, delivering proactive care to residents in our Skilled Nursing and Long-Term Care facilities.

Active Illinois Nurse Practitioner License (unencumbered).
Master's Degree in Nursing from an accredited program.
Strong clinical background in primary care and preventive medicine.
Geriatric or adult medicine experience preferred, with a focus on skilled nursing, long-term care, hospice, palliative care, or similar settings.
Ability to engage and communicate effectively with elderly and chronically ill patients and their families.
Ability to maximize onsite time to develop robust caseloads
Knowledge of Medicare, Medicaid, and Health Plan benefit structures is a plus.
Ensuring continuity of care by communicating with the primary In-House Nurse Practitioner
Conducting new admission evaluations and monthly stability visits for long-term residents
Performing comprehensive physical examinations and evaluating changes in patient conditions
Prescribing medications and treatments as needed
Developing patient care policies and facilitating referrals to specialists
Coordinating advanced care planning and discharge visits
Ensuring timely communication and documentation of patient assessments, treatment plans, and follow-ups
Supporting the physician-patient relationship and addressing gaps in care
Our NPs work closely with the interdisciplinary team, taking a leadership role in patient care, collaborating with physicians, and ensuring high-quality service delivery in our facilities.
