The Nurse Practitioner or Physician Assistant – Advanced Wound Care role is a full-time, on-site position based in Illinois, United States. In this role, the clinician performs comprehensive wound assessments, develops and implements evidence‑based treatment plans, and provides ongoing evaluation of healing progress. Daily responsibilities include coordinating care with referring providers, documenting visits and treatment outcomes in accordance with regulatory and payer requirements, and educating patients and caregivers on wound prevention, dressing changes, and overall health management.

The clinician will travel to patient homes or designated care sites, maintain strict infection control standards, and contribute to quality improvement initiatives that enhance patient outcomes and service delivery.

• Active Nurse Practitioner or Physician Assistant license in Illinois, with appropriate national certification and in good standing.
• Clinical expertise in wound care, including assessment, debridement, dressing selection, infection management, and chronic disease considerations.
• Strong patient education and communication skills, with the ability to explain treatment plans clearly and collaborate effectively with interdisciplinary teams.
• Proficiency in clinical documentation, EMR/EHR systems, and adherence to regulatory, billing, and compliance standards.
• Demonstrated ability to work independently in the field, manage time efficiently, and adapt to varied home and facility environments.
• Commitment to patient‑centered care, cultural sensitivity, and maintaining professional boundaries and ethical standards.
• Current BLS (and preferably ACLS) certification and valid driver’s license with reliable transportation.
• Prior experience in wound care, home health, geriatrics, or acute care is highly preferred.
