Nurse Navigator, REI Clinic: Patient Care & Education

UChicago Medicine is seeking a Nurse Navigator to collaborate with the REI clinic across the South Loop and Hinsdale locations. The role coordinates care, educates patients, and supports discharge planning and transitions of trauma/OON patients within the network.

You will liaise with multidisciplinary teams to address clinical, emotional, financial, and other patient needs. Ideal candidates will hold an Illinois RN license with at least two years of nursing experience and strong communication
