Overview
Registered Dietitian — Skilled Nursing Facility We are seeking a compassionate and detail-oriented Registered Dietitian to join our skilled nursing team. The Dietitian will assess residents’ nutritional needs, develop individualized care plans, monitor weight and intake, and collaborate with nursing and therapy teams to support resident health and recovery.
Responsibilities- Perform nutrition assessments for admissions and ongoing residents
- Develop and monitor nutrition care plans
- Track weight changes, labs, intake, and hydration
- Participate in care plan meetings
- Support regulatory compliance and documentation
- Registered Dietitian (RD/RDN)
- State license (if applicable)
- Long-term care experience preferred but not required
