Requirements:

• Current state license as a Registered Nurse
• 1-2 years of experience as a Registered Nurse in a clinical care setting
• 2+ years of home health experience
• Homecare Homebase Software experience
• OASIS experience
• CHPN certification (preferred)
• Proof of current CPR certification
• Current driver's license
• Reliable transportation and insurance

Roles & Responsibilities:

• Assess home health and hospice patients to identify physical, psychosocial, and environmental needs.
• Implement, develop, document, and evaluate the Plan of Care (POC) to ensure quality and continuity of care.
• Educate patients and families regarding care, medications, and safety, promoting self-care practices.
• Supervise and provide clinical direction to assistive personnel to ensure quality service delivery.
• Perform on-call responsibilities and provide effective communication to patients, families, and healthcare teams.
• Attend interdisciplinary team meetings and other necessary meetings as required.

Compliance / Notes:

• Must comply with OSHA infection control measures.
• Maintains current licensure and continuing education requirements.
• Ability to work a flexible schedule and travel locally.
• Confidentiality must be maintained.
