Gentiva Hospice is seeking a dedicated RN Case Manager to deliver compassionate end-of-life care in patients’ homes and other settings. You will conduct comprehensive assessments, develop individualized plans, and work with physicians, social workers, and other team members to meet patient goals.
You will educate patients and families on hospice services, symptom management, and care planning, while maintaining accurate documentation and complying with regulations.
