Visit NP: Comprehensive Geriatric Assessments

Matrix Medical Network seeks a Nurse Practitioner to perform Comprehensive Health Assessments in patients’ homes or Skilled Nursing Facilities. Visits can last up to an hour, covering medical history, medications, adherence, and social factors to gain a full view of a member’s health.

You will conduct adult/geriatric assessments, order and interpret tests as needed, and collaborate with PCPs and care teams to close care gaps.
