Omega Foundation Services, Inc is looking for Form Carpenter(s) to join the team!

The Heavy Civil Form Carpenter is responsible for constructing, installing, and removing concrete formwork used in heavy civil construction projects. This position works on bridges, culverts, retaining walls, foundations, water and wastewater facilities, roadways, and other infrastructure projects by building precise form systems that meet engineering specifications and quality standards. The Heavy Civil Form Carpenter works closely with project supervisors, ironworkers, concrete crews, and equipment operators while maintaining a strong commitment to safety and craftsmanship.

• Construct, assemble, install, and dismantle wood, plywood, steel, and modular concrete formwork.
• Build forms for footings, foundations, walls, columns, bridge components, retaining walls, box culverts, headwalls, spillways, and other concrete structures.
• Read and interpret blueprints, shop drawings, specifications, and layout plans.
• Measure, cut, shape, and assemble construction materials using hand and power tools.
• Install embeds, anchor bolts, blockouts, keyways, chamfers, expansion joints, waterstops, and other concrete accessories.
• Verify dimensions, elevations, alignment, and plumbness of forms before concrete placement.
• Assist with reinforcing steel installation and coordinate with rebar and concrete crews.
• Strip and remove formwork after concrete has cured while protecting finished structures.
• Perform layout using levels, lasers, measuring tapes, string lines, and surveying equipment.
• Build temporary structures such as work platforms, stairs, handrails, and safety barriers.
• Safely operate carpentry tools and construction equipment.
• Maintain clean and organized work areas to promote a safe and productive jobsite.
• Inspect formwork for quality and compliance with project specifications before concrete placement.
• Follow all OSHA, company, and project-specific safety procedures.
• Assist with general construction tasks and support other crafts as project needs require.
• Perform other duties assigned by supervisors.

• High school diploma or GED preferred.
• Minimum of 3 years of experience as a form carpenter in heavy civil, highway, bridge, or industrial construction.
• Experience with wood, gang, steel, and modular forming systems.
• Ability to read and interpret construction drawings and specifications.
• Knowledge of concrete placement procedures and forming techniques.
• Strong mathematical and measuring skills.
• Valid driver's license preferred.

• Ability to lift and carry up to 75 pounds.
• Ability to stand, walk, bend, kneel, climb, crouch, and work on uneven terrain throughout the workday.
• Ability to climb scaffolding, ladders, and work at elevated heights.
• Ability to work in trenches, excavations, and confined spaces when required.
• Manual dexterity to safely use hand and power tools.
• Ability to work outdoors in varying weather conditions.

• Outdoor construction sites with exposure to dust, mud, noise, heavy equipment, vibration, and changing weather.
• Frequent work around cranes, excavators, concrete pumps, and other heavy machinery.
• Overtime, weekends, and travel may be required depending on project schedules.

• OSHA 10-Hour or OSHA 30-Hour Construction Certification
• First Aid/CPR Certification
• Competent Person – Excavation and Trenching
• Scaffold User Certification Rigging and Signal Person Certification
• Aerial Lift Certification
