Home Health Physical Therapist - per visit

Overview

BAYADA Home Health Care is seeking a Home Health Physical Therapist for a weekend home health role in Southborough, Massachusetts. The position offers flexible per diem, part-time, or full-time scheduling with premium pay and benefits. The role includes work in home settings and surrounding towns (Marlborough, Stow, Acton, Boxborough, and nearby areas). Prior home health experience is preferred but not required.

An experienced, multidisciplinary care team provides skilled nursing and rehabilitative services to adult and geriatric clients in their homes.

• Make visits to clients in their homes.
• Perform diagnostic tests and measurements (mobility, joint range of motion, transfer status, ambulation patterns, strength and endurance, balance, safety).
• Develop and implement individualized care plans, including manual therapy, gait training, balance interventions, and other therapies.
• Continuously assess and revise the Physical Therapy care plan and participate with nursing in the multidisciplinary care plan as appropriate.
• Educate clients, families, or other representatives on rehabilitative care to promote health, safety, and independent living.
• Accurately document observations, interventions, and evaluations using appropriate documentation tools.

• A current license as a Physical Therapist in Massachusetts.
• A minimum of one year of recent clinical experience as a licensed Physical Therapist.

Base Pay: $64.00 - $72.00 per hour, depending on qualifications, plus weekend differential.
