VNA Care Home Health RNs

VNA Care is seeking Home Health RNs for full time roles.

Schedule:

Monday - Friday 8am-4:30pm. No weekends!

Territory:
Framingham, Natick and surrounding towns.

Salary range: $86,000 - $129,000 for a full-time schedule, would need to adjust for a part time schedule.

Qualifications:

• Must hold a valid MA RN License in good standing
• Minimum of 1 year of med/surg experience; acute care experience is beneficial
• Ability to work with compassion, organization, and excellent multitasking skills
• Willingness to travel within a designated territory

Responsibilities:

• Conduct home visits to assess patient needs and provide necessary care
• Utilize Case Management, Oasis E documentation, Time Management, and EMR system, Home Care Home Base
• Collaborate with patients, families, and interdisciplinary team members to develop and implement care plans
• Ensure accurate documentation of patient progress and treatment plans
