Job DescriptionThe Transition Coordinator is responsible for the overarching management of transition supports and programs offered to students ages 18-22 years old in the Southbridge Public Schools. Specifically, the Transition Coordinator will oversee the supervision and evaluation for the Job Coaches. Further, the Transition Coordinator will engage in ongoing management of transition assessment tools, conduct Transition Assessments as needed, work with IEP teams to develop goals and objectives for transition, and develop comprehensive transition plans for IEPs.
The Transition Coordinator acts as the liaison for outreach to families, community based organizations, and state agencies, to ensure seamless and holistic transition planning for students who are in the process of post‑secondary transition planning (ages 18‑22). Further, the Transition Coordinator will oversee the development, coordination, and support of individualized transition services for students with disabilities focusing on preparing students for education, employment, independent living and community participation in accordance with their IEPs and MA State Guidelines.
- The Transition Coordinator will serve as the key project manager for all Transition‑related activities and programs as well as an important point person for each young adult and their families as they navigate their journey from the Southbridge Public Schools.
- Coordinate process for IEP goal development and implementation, including the collection of data for IEPs and progress monitoring.
- Oversee a caseload of students (ages 18‑22) providing support to staff and direct instruction to students as needed in the areas of life skills, vocational exploration, career readiness and community participation.
- Participate in IEP meetings to support the transition planning process.
- Oversee the supervision and evaluation of Job Coaches.
- Act as the liaison between the Southbridge Public Schools, state agencies, community partners and families to facilitate discussions about transition planning and services.
- Develop comprehensive transition program through lines that outline process for student transitions in collaboration with high school team.
- Provide consultation and problem‑solving as it relates to the daily oversight of all transition department programs and staff.
- Create and oversee a database of community partners including but not limited to college student support offices, community based day programs, day habilitation programs, state agencies (MRC, DDS, MCDHH, MCB, DCF and DMH), vocational partners, etc.
