RVNAhealth in Connecticut is seeking a Registered Nurse (RN) Hospice Case Manager to join our patient-centered hospice team. You will manage a caseload and coordinate care across the interdisciplinary team in patients' homes and community settings, ensuring comfort and dignity at end of life.
The role requires a CT RN license (or Compact), BSN preferred, and at least 2 years of nursing experience with hospice or palliative care preferred. Travel throughout Southern Fairfield County is expected.
