What is Take Your Therapist Home?
In a world of shorter and shorter SNF stays, Take Your Therapist Home (TYTH) is a one‑of‑a‑kind program where you as the clinician work directly with patients in the SNF and home health, providing continuity of care that allows you to have a greater impact on patient outcomes! You will be primarily in the home health setting with dedicated time and caseload support for the SNF, covering a localized area around the SNF.
Your schedule is based on a traditional work week.
Rivetus has been serving the geriatric Post‑Acute community since 1998. We are a specialized rehabilitation company operated and managed at the executive level by licensed clinicians, offering continuity of care across Skilled Nursing, Home Health, and Outpatient settings.

Role Details: This is a hybrid role providing Home Health therapy services in conjunction with SNF‑based therapy services. Home Health geographic area is the vicinity around Evergreen Health & Rehabilitation Center. Home care services are paid per visit, and SNF services are paid at an hourly rate.
Schedule: Monday‑Friday, Full Time – Flexible Schedules Available
Required Qualifications- Successful completion of an accredited Physical Therapist program.
- Licensure as required by the State of Michigan.
- Ability to exercise professional judgment and initiative when analyzing problems and recommending solutions.
- Initiative and flexibility in the work environment.
- Ability to react effectively to the needs of patients and their families.
- Physical ability to bend, position, transfer, and lift patients weighing from 80 pounds to over 300 pounds without restriction, utilizing lifting devices clinically recommended for the patient.
