The Home Health Intake Coordinator ensures that the patients receive the care they need in a timely manner. They are responsible for the timely and accurate entry of a variety of data for the intake of patients. The Home Health Intake Coordinator facilitates patient admission to home care services by coordinating with, but not limited to, interdepartmental staff, other department staff, patients, families, and company community partners.
Additionally, there are times they may assist patients with coordinating future appointments, procedures, or treatments if required by patient conditions.
In this role you will be responsible for:
- Assists in coordinating Home Care Services for patients, in a timely manner.
- Assists the functions of patient care management through timely and accurate intake data entry and system reporting.
- Maintain a good working rapport with community and company partners, interdepartmental personnel, as well as other departments within the facility.
- Audit new admission paperwork to ensure a complete medical record.
- Make welcome calls to patients/family to confirm information is correct.
- Accurately enters, and updates where required, a variety of patient data (both new and previous) and patient care data into information systems.
- Verifies accuracy of patient referral information and data entered through use of information systems reports.
- Assist in the scheduling of patient care activities where appropriate.
- Performs daily backup of information in the system(s) where required.
- Alerts appropriate management team members regarding late or missing documents required for intake of a patient and data entry.
- Alerts appropriate members of the information system support staff to resolve system problems that negatively impact the speed and accuracy of intake and data entry functions.
- Performs other specific projects relating to intake and data entry and computer operations as required.
- Provide clerical assistance where requested.
- Maintains confidentiality of patient and organization information at all times.
- High School Diploma or GED
- One year of experience in health care data entry
- Demonstrated ability to work collaboratively as part of an interdisciplinary team
- Excellent verbal and written communication skills
- Strong organizational abilities and attention to detail
- Associates Degree preferred
- Experience in home care or hospice care information systems preferred

$21.50—$25.50 USD
