Position Overview:
PAY RATE BASED OFF PATIENTS SEEN, NOT HOURLY RATE
We are seeking a skilled and compassionate Nurse Practitioner (NP) to join our team. The NP will provide primary and urgent care services through home visits, collaborating with physicians and other healthcare professionals to ensure continuity of care. This role is ideal for an independent, motivated clinician who thrives in patient-focused environments and values building strong relationships with patients and their families.
Key Responsibilities:
- Conduct in-home patient assessments, examinations, and evaluations
- Diagnose, treat, and manage acute and chronic conditions
- Prescribe medications and order diagnostic tests as appropriate
- Develop and implement individualized care plans
- Coordinate care with physicians, specialists, and other healthcare providers
- Provide patient and family education on treatment plans, medications, and preventive care
- Maintain accurate and timely medical documentation in the EMR system (Glace
EMR) - Adhere to clinical protocols, professional standards, and regulatory requirements
- Travel to patients’ homes within assigned service areas
Qualifications:
- Current and unrestricted Nurse Practitioner (NP) license in Michigan
- Board Certified in Family Practice, Adult-Gerontology, or related specialty
- Current DEA license (or ability to obtain)
- CPR/BLS certification
- Strong clinical, diagnostic, and decision-making skills
- Excellent communication, interpersonal, and organizational skills
- Valid driver’s license, reliable transportation, and willingness to travel
