Home Care Clinical Manager

We are seeking a dynamic and experienced Homecare Clinical Manager - Registered Nurse to lead our clinical team. This vital role involves overseeing patient care delivery, managing nursing staff, ensuring compliance with healthcare standards, and promoting excellent patient outcomes.

Position Highlights:

• Review and coordinates case referrals. Reviews available patient information related to case, including disciplines required, to determine home care needs. Supervise Intake to make sure appropriate clinicians are assigned to case.
• Facilitates, coordinates and integrates orders from all physicians.
• Reviews and evaluates each case by reviewing the services provided by clinicians, conferences, record review, discusses and verifies impressions, instructs and guides clinicians to promote more effective performance and delivery of quality home care services.
• Reviews and critiques all clinical staff documentation. Performs QA & review coding.
• Assists in the screening and interviewing process of new agency personnel and makes recommendations for employment of individuals. Assists in the orientation of new agency personnel.
• Assists Clinical Director in the planning, implementation and evaluation of in-service and continuing education programs.
• Participates in agency Quality Assessment and Performance Improvement Projects as assigned.
• Serves as a backup support for Director of Clinical Services.
