Empatia seeks a Nurse Practitioner, Palliative CPOM, to provide patient-centered palliative care across home-based settings. You will develop and manage comprehensive treatment plans, perform diagnoses, treatments, and follow-ups, and work under the Palliative Medical Director when required by state law.
Required MSN or equivalent, 2+ years NP experience (Family/Adult preferred), and proficiency with EMR systems. Willingness to travel 10-20% for ongoing training and education is essential.
