Homecare Licensed Practical Nurse

The Homecare Licensed Practical Nurse (LPN) plays a critical role in delivering high-quality nursing care to patients within their homes, ensuring their health, comfort, and safety. This position involves assessing patient conditions, administering medications, and providing wound care while closely monitoring patient progress and reporting changes to supervising healthcare professionals. • Current and valid Licensed Practical Nurse (LPN) license in the state of practice within the United States.

• Basic Life Support (BLS) certification.
• Minimum of one year of clinical nursing experience, preferably in home health, long-term care, or related settings.
• Ability to pass background checks and meet health screening requirements.

• Experience working in home health care or community-based nursing.
• Certification in wound care or chronic disease management.
• Familiarity with electronic visit verification (EVV) systems.
• Strong knowledge of infection control practices and patient safety protocols.
• Additional certifications such as IV therapy or geriatric nursing.

• Conduct comprehensive patient assessments and document health status, vital signs, and symptoms accurately.
• Administer prescribed medications and treatments in accordance with established protocols and physician orders.
• Provide wound care, catheter care, and other nursing procedures as required by patient care plans.
• Educate patients and their families on disease management, medication adherence, and health maintenance.
• Communicate effectively with physicians, registered nurses, and other healthcare team members regarding patient progress and any changes in condition.
• Maintain detailed and timely patient records in compliance with regulatory standards and agency policies.
• Ensure patient safety by identifying potential risks and implementing appropriate interventions in the home environment.
• Support patients with activities of daily living as needed, promoting dignity and independence.
