CNA

Job Responsibilities
  • Provides personal care and housekeeping assistance to our clients in their homes; follows the established plan of care under the direction of the client care coordinator.
  • Provides personal hygiene and grooming care to our clients.
  • Provides meals for clients' nutritional needs.
  • Reports changes in clients' condition to client care coordinator or supervisor.
  • Performs routine housekeeping and home management duties.
  • Records each client's care on care plan and submits care plan in a timely manner.
  • Responds sensitively to the multiple, complex needs of a terminal client which includes providing end of life care.
  • Assists with routine therapy exercises.
  • Participates in case conferences as requested.
Qualifications
  • Education:

    High School diploma or equivalent, preferred.
  • Licensure/Certification: CNA Certificate approved by the Connecticut Department of Public Health and Addiction Services required.
  • Experience:

    Six months experience as Home Care Aide/CNA in a home care agency, preferred.
