Location

81 Meriden Ave Bradley Memoria (10003)

Shift

Weekend Sat / Sun 7:30a-4:00p, including holiday coverage for MLK, Christmas, Thanksgiving, New Year's Day, 4th of July, Labor Day and Memorial Day.

Overview

Hartford Health Care at Home is the largest provider of homecare services in Connecticut. As a Team Coordinator, you will be responsible for scheduling patient visits to provide outstanding service delivery while improving the patient experience.

Responsibilities
  • Perform duties and responsibilities with compliance to regulatory agencies as evidenced by ability to apply Agency policies, resulting in deficiency free surveys.
  • Schedule nursing, therapy & social work visits.
  • Facilitate the timely initiation of care through problem solving, accurate scheduling and tasking of requested services along with a critical thinking approach to work.
  • Triage incoming calls using critical thinking, prioritization and superior customer service skills.
  • Coordinate and consult with Supervisors and team members regarding patient information, data management and regulatory compliance statuses.
  • Utilize and manage electronic and paper data and coordinate specific administrative/organizational functions to ensure accurate, efficient, and timely flow of information and financial data for interdisciplinary care teams.
  • Provide administrative and clerical support to the care management team electronically and manually.
  • Demonstrate proficiency in computer navigation and medical/financial databases communications.
  • Electronically assign and disassign Clinical Staff to patients by accurately utilizing system permissions.
  • Coordinate initial admission services and act as liaison for clinical team.
  • Demonstrate effective communication, organizational and time management skills.
  • Comply with agency established processes to maintain standards and ensure standardization.
  • Organize daily work and information to maintain efficiency and confidentiality.
  • Ensure required information is provided and flows timely to complete the functions of billing, payroll, admissions and care delivery.
Qualifications

Education: High School diploma or equivalent;
Associate's Degree with medical secretarial background preferred.

Experience: Three years of related office experience in a health-related field preferred.

Skills: Office procedures, computer proficiency.
