The Hospice Visiting Nurse (RN) provides personalized care to patients in their homes during their end‑of‑life journey. This role is ideal for compassionate nurses who want to make a meaningful impact in patients’ lives.

• Support patients, families, and loved ones during their end‑of‑life journey.
• Perform comprehensive assessments to establish goals for symptom management and end‑of‑life care.
• Be a member of an active interdisciplinary team addressing patients’ needs holistically.
• Develop and maintain therapeutic relationships through effective communication with patients and families.
• Utilize state‑of‑the‑art technology to document skilled services provided.

Education: Registered Nurse with Valid CT RN license

Experience: One year of RN experience;
Homecare or Hospice experience preferred.

Licenses &

Certifications:

Valid driver’s license and reliable transportation required. Computer proficiency required.
