About This Role
The Hospice Admission Nurse (RN) plays a crucial role in providing personalized care to patients at the end of life, supporting them and their families through this journey. - Department/Unit: 95005-Southington Hospice Gen
- Schedule: Full Time, Day shift
- Support patients, families, and loved ones during their end‑of‑life journey.
- Perform comprehensive assessments to establish goals for symptom management and end‑of‑life care.
- Implement effective case management strategies.
- Collaborate as a member of an active interdisciplinary team to address patients’ needs holistically.
- Develop and maintain therapeutic relationships through effective communication with patients and families.
- Utilize state‑of‑the‑art technology to document skilled services provided.
Registered Nurse with Valid CT RN license
Experience1+ year of RN experience;
Homecare or Hospice experience preferred
- Valid driver’s license and reliable transportation required
- Computer proficiency required
