As part of a person-centered care model, and under the direction of the RN, this role will provide skilled nursing care to clients in home and community-based settings ensuring the delivery of outcome based, high quality, cost-effective care. This role will collaborate with all team members to achieve exceptional outcomes.
Provide intermittent skilled nursing services in accordance with the established plan of care under the direction of an RN.
Provide support to patient/family utilizing teaching principles to promote and restore health and prevent disease and disability.
Record daily progress in EMR.
Report to primary nurse, supervisor, physician, social worker, or therapist regarding cases.
Communicate/conference with primary nurse and other disciplines.
Consult with physician regarding client needs and plan of care.
Assist in evaluation of care given by paraprofessional staff.
Participate in agency committees and special projects.
QualificationsMinimum Requirements:
- High School diploma or GED
- Successful completion of a CT state approved LPN certificate program
- Experience
- Computer skills including but not limited to Outlook, Microsoft Word, and Excel
- Licensure
- Current and valid LPN license
- Valid CT driver's license & a good driving history
