Mercy South Hospice is seeking an Intake RN to manage referrals and coordinate care across Hospice, Home Health and Home Infusion services in the St. Louis region. This PRN position supports discharge planning and collaborates with physicians and social workers to ensure timely patient placement.
The ideal candidate has an RN license with 3+ years of nursing experience, plus Hospice/Home Health expertise. Bachelor’s degree preferred; CHPN certification a plus.
