Pay Range: $18.50 – $20.50

Scheduled Shift: 6 pm – 6 am
Job SummaryThe Med Care Manager/QMAP must administer, pour, pass, and document all routinely prescribed medications per physician orders under the direction of the Wellness Director and as approved by the State Medication Administration Regulations. The role requires learning each resident’s medication preferences and ensuring timely delivery and administration.

- Administer and document regularly prescribed medications per the Medication Administration record, observing the five rights for safe administering of meds and per physician orders.
- Order meds from the pharmacy when needed and 7 days before running out; notify the Wellness Director within 24 hours if meds do not arrive.
- Document medication administration immediately after administering and record vital signs as ordered.
- Observe and report reactions and side effects to the licensed nurse.
- Administer PRN medications per physician order.
- Report any medication errors to the Wellness Director immediately.
- Assist with development and review of the resident’s care plan with other disciplines.
- Ensure residents’ privacy, respect, and dignity.
- Detect, correct, and report unsafe conditions.
- Assure physical comfort, safety, and mental well‑being of residents.
- Assist in daily care tasks: toileting, grooming, bathing, feeding, dressing, monitoring intake, hair & nail care, moving residents, and responding to call lights.
- Demonstrated core values:

State‑approved Medication Administration training, First Aid certification (as required by ALF regulations), QMAP in Colorado. - Education/
Experience:
Minimum high‑school diploma or equivalent; prior experience in Assisted Living or Memory Care, with medication administration experience. - Continuing education: required by law and participation in monthly in‑service training.
- Other:
Ability to read and write English; strong communication skills; good judgment and responsibility for resident care. - Job Knowledge:
Medication administration procedures, universal precautions, and infection control. - Industry
Experience:
Minimum three years’ experience in senior living preferred. - Team members working 24+ hours per week and their families are eligible for benefits.
