This role is a part-time Rehab Nurse or LPN position at a new acute rehabilitation hospital. The nurse will provide patient-centered, compassionate care in a safe environment, emphasizing service excellence. Responsibilities include
- Assessing patients' psychosocial, emotional, and spiritual needs
- Developing and implementing individualized care, treatment, and discharge plans
- Conducting purposeful hourly rounds to ensure safety, comfort, and safety
- Collaborating with patients, families, and the interdisciplinary team
- Following effective communication practices like SBAR to ensure seamless care transitions
