Licensed Practical Nurse

Standing Stone Care and Rehab Center in Cookeville, TN is seeking a Licensed Practical Nurse to deliver resident-centered care in our skilled nursing and rehab facility. The LPN will provide bedside nursing, administer medications and treatments, monitor vital signs, and document care accurately in accordance with care plans and regulatory standards. Working closely with RNs, therapists, and physicians, this role supports rehab and long-term care residents, educates families, and helps maintain a safe, homelike environment while upholding high standards of compassion, teamwork, and clinical quality.

Responsibilities

• Provide direct bedside nursing care to rehab and long-term care residents
• Administer medications and treatments per physician orders and facility policy
• Monitor, record, and report vital signs, changes in condition, and resident responses
• Document care accurately in electronic or paper medical records
• Follow individualized care plans and regulatory standards for skilled nursing
• Collaborate with RNs, therapists, physicians, and CNAs to coordinate care
• Educate residents and families on care, treatments, and health routines
• Support safe infection control, fall prevention, and quality assurance practices
• Participate in care conferences and interdisciplinary team meetings
• Promote a compassionate, respectful, and homelike environment for residents
