Registered Nurse

Standing Stone Care and Rehab Center in Cookeville, TN is seeking a compassionate Registered Nurse to provide skilled nursing care for short-term rehab and long-term care residents. The RN will assess resident conditions, develop and implement individualized care plans, administer medications and treatments, and collaborate with an interdisciplinary team to support optimal outcomes. Responsibilities include monitoring changes in condition, documenting accurately, supervising CNAs, and educating residents and families.

Responsibilities

• Provide skilled nursing care to rehab and long-term care residents
• Assess, plan, implement, and evaluate individualized care plans
• Administer medications and treatments per provider orders
• Monitor and document resident status and report changes promptly
• Collaborate with interdisciplinary team to optimize outcomes
• Supervise and support CNAs and other support staff
• Educate residents and families on care plans and health management
• Maintain infection control and safety standards
• Ensure accurate, timely clinical documentation
• Support a compassionate, resident-centered environment
